The Polk County Board of Commissioners is proud to honor and appreciate those who  have made a  positive contribution to our County. If you know someone who has shown and exceeded being a role model or has volunteered or demonstrated a selfless act for the county, please fill out the nomination form below. The nominations start each year on January 1st and end October 31st of each year.

No candidate shall be discriminated against because of race, creed,color, age, sex or religion. Only one nomination per citizen is allowed per year.

The selected nominee will be presented at the Polk County Commission Meeting held on the 1st meeting in December. An award and plaque will be presented during the meeting.
